Surveyor Questions

What is the procedure for a medication error of ADR? He mentioned the response that he received from Oncology and asked the Pharmacy Director if this was correct and did it happen that way every time.

Should the Pharmacy be notified of a medication error always whether the status is inpatient or outpatient?

Should we trend medication errors?

Should physician who prescribed medication be notified of error in dosage, prescription errors, etc.?

Who is responsible for the assessments of these type things?

Don’t we as the hospital pharmacy department have a responsibility for these type of assessments?

If the prescription is written from here then should we be notified if an error or reaction occurs?

If the prescription was filled by a retail pharmacy should the hospital pharmacy still be notified?

Should errors of this nature be trended and reported?

Are PharmD’s the future?

The system we currently have in place, how long has it been here and what was the expense involved?

Who do you contact for poison control questions/issues?

When is the hood surveyed and how often is it done?

Are cultures done anymore back in the pharmacy?

Are there any problems with Schedule 2 meds?

Are experimental meds done here?

If a patient is on an experimental medication when admitted into hospital, what is the procedure from here and who takes responsibility for making staff aware and the education of the medication?

Have we ever had a patient on an experimental meds not approved by the FDA?

Do you allow patients to bring medications from home?

Do you ever have patients who refuses to receive medication from the hosptial and insists on using medications brought from home?

Noticed that he tried to pit Oncology answers against Pharmacy answers.

Noticed he pried too much into personal things, even though I am sure it was only used as a distraction technique.

Still using the technique of asking the same questions 2 or 3 times throughout interview in hopes of getting different answers.
