Medical Staff Services

After document review asked for files on 25% of total medical staff. Surveyor chose file by name and specialty.

Cross-checked files against Bylaws and Regs.

Looked hard for retroactive appointments and SE data.

Medical Record Interview, closed

Charts selected by us against the standard criteria. 20 restraint charts and 1 autopsy chart.

Once again I was amazed that we cannot accurately audit and score our own charts. This session is like correcting your own open-book exam and only with the surveyor’s assistance and education were we able to avoid a Type 1.

1 incorrect restraint chart/20 = score of 2
