| A Little About Your Host |

To introduce myself, I am a Registered
Nurse of 30 years. My experience includes Medical/Surgical Nursing, Critical Care,
IV Therapy of 10 years, Home Health Care, and 18 Years in the Quality arena!! I am
currently employed at a pediatric facility. I have experienced several Joint Commission surveys,
and have provided consulting services. I am active in my regional and national associations for healthcare
quality. I have been a Certified Professional in Healthcare Quality since 1987. I believe in functionality...so you probably will not find anything fancy
here...but instead, a purposeful and functional web site! But from time to time if I
get bored or creative, you may find a new graphic or other web item popping up! And
please, use the comments page to let me know what you think about the site. Share
your ideas or suggestions with me to make the site a place you can use and enjoy.
Please remember, I'm not a professional web designer...just a Health Care professional
like you. But I am willing to advance my skills and knowledge to meet your needs.

| How To Submit Documents For Sharing: |

I hope that you will not
only take from this site, but that you will also take some time to share your valuable
expertise with others by submitting items to be posted here for sharing.
- When possible, please submit your items in a universal word
processing or text file format.
- You can transfer the files to me as an attachment or as part of an
email message. If it is necessary to fax the document or send it via snail mail,
please contact me via email for further information for doing so.
- Please remove any information that would identify your facility
before sending the file. If identifiers are found in the document, they will be
removed. Consultants that submit documents will be acknowledged for their
contribution.
- When submitting documents for publication on this web site, please
include the following information (Your name and facility will remain
confidential. It will not be posted to the site):
- Your name and email or other contact information
- The name of your facility
- Type of facility
- Function as it relates to the Joint Commission's key functions
(outlined below)
- Date of your last Joint Commission survey
- Any comments that you would like noted along with the document you
are submitting

| How To Find Your Way Around The Site |

The site is organized by function,
following the Joint Commission's 11 key functions. The functions are listed on the
"Links To Documents Page" for those who may not be
readily familiar with the abbreviations noted in the navigation bar at the bottom of each
page. By clicking on the function, you will link to another
page that will provide information that other Health Care Professionals have so graciously
been willing to share regarding that function. |
| Document Numbering |
|
Each document submitted has been numbered
using the following numbering convention. |
|
Each document will be assigned a code for
easy identification of the document type. The code assignment will be as follows: |

Will provide an
capitalized alphabetic character to represent the function as indicated by the Joint
Commission function |
| xxx |
Will provide a
numeric document number. If more than one related document is submitted by the same
organization the same number, xxx will be assigned to all of the documents. |

Will represent a
capitalized alphabetic character indicating the type of document.
P=Policy or Procedure
F=Form
L=Plan
O=Other Document |

Will represent a
final alpha character that will be provided if more than one of the same document type is
submitted by the same facility relating to the same function in order to relate necessary
documents to each other. |

| Comments |
|
Each document will have a
"Comments" section. When comments have been
provided by the individual submitting the document, they will be noted in this
section.
If no comments have been submitted, "No Comments" will be
noted in the comments section. |
| Related Documents |
|
When related documents are submitted, it
will be indicated in the "Related Documents" section. Related documents
will be grouped together alphanumerically. |
| Viewing or Downloading Documents |
|
Links to documents that are available on
this site will be noted next to the document title. The file extension will indicate
the file type. Those with '.exe' extensions can be
downloaded by clicking on the hyperlink. These are self-extracting files. When
you click on the file you will be prompted for a location to save the file. Once
downloaded, double clicking on the file will cause the files to extract. The
individual files can then by opened by the software indicated.
Those with '.pdf' extensions can be viewed online with Acrobat
Reader which is a free downloadable software that can be obtained from Adobe's
website. Just click on the hyperlink here or on the logo below
and follow the directions on their page to download and install the Acrobat Reader.
Once you have downloaded it, you can view and print the forms directly from the web.
Right clicking on these files will bring up an option
menu. To download the file, choose 'Save Link As' and then indicate the location
you would like to save the file to. |

page for consultants. If you are a consultant and have a web page, I will be
glad to add your link to the list of consultants. If you have a logo you can
email it to me to include with the link. In addition, any other information
regarding your consulting service will gladly be included with your link. If you are a consultant and do not have your own web page, I will be glad to
include any information you send me on the consultants web page. |
